Who Staffing is looking for a Care Manager for a fulltime, direct hire opportunity with our client located in either Baltimore, MD or Wilmington, DE or Newark, DE.
Job Summary:
Under limited supervision, the Care Manager is responsible for managing the clinical, medical, and psychosocial care for clients, including vulnerable adults and elderly populations. This position is also expected to support the overall operation of our client as needed. Certain tasks may be required that do not fall into the essential functions of this job.
Job Responsibilities:
· Conduct initial clinical assessments to discern the client situation and inform the family of recommendations and next steps
· Research and identify appropriate care settings and support services needed for clients to ensure proper services are implemented
· Write reports that are clear, accurate, concise and relevant in an efficient manner focusing on outlining the value of a Care Manager
· Advocate on the client’s behalf to receive services, entitlements, benefits or the best care scenario for the client, working collaboratively with State and Local agencies and other external partners
· Coordinate client care functioning as a liaison between client, family, attorney, physician, other services providers, etc., facilitating family meetings and having difficult conversations
· Perform follow-up visits and engagement visits on an ongoing basis to monitor client status
· Administer cognitive screenings and make determinations of competency as appropriate
· Request, collect and secure medical records from various entities maintaining client confidentiality
· Develop and monitor care plans with individuals, their families, and service providers to ensure they are addressing their needs and adjust as appropriate
· Conduct assessments to ensure home environment is safe and supportive of clients’ needs
· Attend clients’ medical appointments, transporting clients as needed, to obtain findings and recommendations, discuss information, collaborate with physician, and communicate outcomes to appropriate individual (family, guardian, other physicians etc.)
· Respond to emergencies in an appropriate manner and notify family, APS, police, physician, or other entities involved. This might include making a client visit outside of work hours
· Guide clients and families through transitions of care between hospital, hospice, care facilities, and appropriate integrated community settings
· Document all case activity including reports, calls, emails, texts, expenses, receipts in the case notes as well as nonbillable time in the case documentation system the same day or no later than end of the week
· Provide family education on a variety of topics including caregiver support services, various medical conditions, differences in levels of care, and benefit navigation
· Purchase and deliver items for clients as needed
· Assist with covering appointments for other care managers as needed
· Direct Care Manager Associate with completing support activity or research needed
· Represent the company at networking and marketing events. This may include visits to referral sources, file reviews with organizations, support groups, dementia training, webinars/presentations, and staffing a table at a marketing event
· Attend all Staff Meetings, monthly Department Meetings, File Review Meetings, and other company meetings as requested
· Comply with company policies, procedures, and regulations
· Perform other duties as assigned

· Ability to perform all job functions efficiently and maintain minimum billable requirements on a consistent basis
· Ability to show compassion and take extra time to hear client concerns, understand nuances of the situation and provide support and/or solutions as appropriate
· Commits to exceeding expectations and delivers the highest level of customer service
· Maintains training, licensures, certifications, etc., required to remain current in profession
· Proficient in Microsoft Office products (Word, Excel, Power Point)
Travel:
· Frequent local travel is required, intermittently throughout the day, depending on the client’s needs
Required Education and Experience:
· Preferred degrees include BSN, RN, MSN, MSW, LCSW or Master’s Degree in a health related or counseling field
· 5 years’ experience in related clinical setting
· 1-2 years of Care Management Experience
· Valid, current state Driver’s License and minimum car insurance requirements
